What is a PCL Injury?

Located in the center of the knee joint, the posterior cruciate ligament (PCL) helps maintain knee stability during everyday and sporting activities. The PCL prevents the tibia (shinbone) from sliding too far backward and keeps the tibia and femur (thighbone) in alignment. It is the strongest ligament in the knee, making PCL injuries less common, accounting for just over 20% of reported knee injuries.

Causes of a PCL Injury

  • Hyperextension: The knee joint is forced backward beyond its normal range.

Symptoms of a PCL Tear

  • Immediate swelling and pain in the knee.

Diagnosing a PCL Injury

Tests Performed:

  • Reverse Lachman’s Test: Evaluates the integrity of the PCL.
  • Posterior Drawer Test: Assesses knee function and mobility.
  • X-rays & MRI: Used to examine bone structures and soft tissue damage.

Grading of PCL Injuries

  • Grade 1: A small, partial tear.
  • Grade 2: A larger but incomplete tear.
  • Grade 3: A complete PCL tear.

Treatment for PCL Injuries

Non-Surgical Treatment

  • RICE Therapy: Rest, Ice, Compression, and Elevation.
  • Physical Therapy: Strengthening exercises for knee stability.
  • Bracing: Provides knee support and prevents excessive movement.

Surgical Treatment

  • Arthroscopic Surgery: Minimally invasive procedure to repair or reconstruct the PCL.
  • Tissue Grafts: Autografts (patient's tissue) or allografts (donor tissue) are used to replace the damaged ligament.
